Referees on the panel and national list should contact PGMO.  For officials entitled to free CRB checks, please do not send in any money.  Your disclosure is now paid for by The FA however if you wish your documents to be returned by Special Delivery you must still enclose a pre-paid Special Delivery envelope.  Please include on line 13 in section B of the disclosure form, where it asks for position applied for, your FAMOA number after entering the word referee.

If your cheque has already been cashed then you will receive a refund via The FA, however the process for this has not yet been finalised. If you are entitled to a free disclosure and have posted us your cheque recently we will return it to you with your documents.
