For the third year running, The FA have teamed up with Sport England and Ipsos MORI to conduct an important survey about your experience of taking part in football and we would appreciate you taking the time to share your views.

The survey will help drive and shape football and sport in England in the years to come as we work to deliver a lasting legacy from the London 2012 Olympic and Paralympic Games.

Sir Trevor Brooking, The FA’s Director of Football Development, says: “By taking part in this survey, you will be helping us to understand more about your experiences, your motivation for taking part in football and the challenges you face.

"This will help The FA to be more responsive to your needs, improve what we offer to existing participants and do more to encourage new ones.”

The survey will establish how satisfied you are with the quality of your sporting experience when taking part in football. Last year 44,000 people told us about their experiences and we are now repeating the survey to ensure we keep up to date with your views.

To become part of this unique project and help shape the future of football in England, please click on the relevant link below – your help and time would be much appreciated by The Football Association and Sport England.

By taking part you will be entered into a prize draw for the opportunity to win a pair of England v Holland tickets for the International Friendly at Wembley Stadium on 10 August, or a signed England shirt! Links to the survey and terms and conditions are set out below.

Please note you must be over 16 to complete the survey and be entered into the prize draw. The survey will be live until 18 May 2011.
