Standard Forms

On this page you can find all the forms and information relating to player registration, dual representation, annual returns, agent registration and payments via The FA clearing house.

AG Forms

The AG1-NR is required when no agent has been used by any party in the deal.

The AG1 form declares all the agents used in the deal and must be signed by all parties.

The AG1/P declares the payments being made by the player to the agent and must be completed if the player has used an agent (even if no payment is being made to the agent).

The AG2/P forms part of the annual returns required to be completed yearly, for every player the agent represents. It details the payments made direct (or through salary deduction) by the player to the agent.

The AG3 is required to be completed by all parties who wish to complete a transaction where there may be a conflict of interest or potential conflict of interest.

When a player is represented by his/her Close Relation in a transaction, the AG6 form needs to be completed to declare that no payment has been made to the Close Relation (as per The FA Football Agents Regulations).

All agents must submit the AG7 form to The FA to allow payments paid via The FA Clearing House to be forwarded.

The AG8 form is part of the annual returns required to be completed yearly be every Licensed Agent. This form contains the agents contact details and organisation information.

The AG/PC form must be completed when a Club wishes to use the services of an Authorised Agent in respect of any Transaction or Contract Negotiation, and the Player that is currently represented by the same Authorised Agent, consents to the provision of services to the Club. This form must be submitted to (with theAG3, if required) and acknowledged by The FA before the club and agent enter into any agreement. The process should be as follows:

  • Player/Agent Representation Contract lodged with The FA;
  • AG/PC form lodged with The FA along with AG3 Conflict of Interest form if applicable, (i.e. in the event that the Player/Agent Representation Contract doesn’t contain the Player Consent clause);
  • The FA to provide acknowledgement of AG/PC form;
  • Club concludes Club/Agent Representation Contract (or Tripartite Agreement);
  • Registration documents (including the Club/Agent Representation Contract) to be lodged in the usual manner

Payments

The FA Football Agents Regulations state that all payments made by a club to an agent are required to be made via The FA Clearing House. Payments to clubs belonging to other national associations in respect of transfer fees, solidarity and training compensation are also required to be made via The FA Clearing House.

When a payment is made to The FA, the payment instruction form below must be completed detailing who the payment is for, what it is in relation to and the bank details for the beneficiary.

Solidarity Calculator

When a player is transferred between two clubs in different countries, 5% of the transfer fee paid is due to the clubs who trained the player between the ages of 12 and 23. The below calculator can help to determine the amount due to these clubs. A completed version must be submitted whenever a solidarity payment is made via The FA Clearing House.
