Consent forms and safeguarding guidance
Before you can take part in Football Mash Up, a few forms need to be completed and returned to your FA Vauxhall Youth Officer. If you have any questions relating to these forms, please email your relevant FA Vauxhall Youth Officer using the addresses below:

Forms to be completed:
1) Parental Information and Consent – This document needs to be completed and emailed to The FA Vauxhall Youth Officer prior to the start of the first session. Please see above for your relevant regional email addresses to send to.
2) Parental consent form - this needs to be signed by a parent or guardian of the Football Mash Up participant. This needs to be signed and handed to your FA Vauxhall Youth Officer at the start of the first session.
3) Player commitment – to ensure all participants get the most out of Football Mash Up, players are asked to sign a commitment form. This needs to be signed and returned to your FA Vauxhall Youth Officer at the start of the first session.
4) Photo and video consent form – occasionally, the FA will take photos and video content for use on the website and promotional materials. All the terms and conditions are clearly defined on the form. This needs to be signed and returned to your FA Vauxhall Youth Officer at the start of the first session.
