- Produced through the FA Medical Committee 7 years ago for use by Professional Club Medical Staff for professional players on request from The Leagues and supported by The PFA.
- Cost of production £45,000 paid for by The Football Association.
- 5 Medical Records Academy advisory visits organised by The FA Head of Medicine and Exercise Science
- Carried out over the last 5 years. Good Feedback Medical Records being used to good effect.
- FA Medical Record Folders- revised and refined this season. All funded by Premier League and Football League.
- The cost of re-design was funded by The Football Association = £1,400

Note: The FA Premier League and Football League Regulations state that the buying club should receive the medical records of the player involved in a transfer following a request by the "signing" club.
