Just a reminder that the deadline to submit your 2007-08 FAMOA Incentive Scheme log book along with your completed order form is Thursday 31 July 2008.

We regret that log books received after this date will not be processed. Please ensure that you fully complete both sides of your order form and fill out all parts of the log book correctly before sending it in. Failure to do so will result in your log book being returned to you. It is advisable that you make a photocopy of your log book as a back up should it get lost in the post.

You can monitor the progress of your claim by logging in to the website www.famoa.com/incentive. Here you can also view images of the items available to redeem your points for and track your order.

If you are an existing member of the Incentive Scheme, membership rolls over from season to season so there is no need to re-register annually. The 2008-09 log books have now been printed and are in the process of being sent out to members.

If you are not currently a member of the scheme and would like to become one, please email famoa@thefa.com or call 0207 745 4651 requesting to join. On becoming a member, a log book and letter explaining the scheme in full detail will be posted to you in due course.
